ABSTRACT

Transforming growth factor-β1 (TGF-β1) acts as a tumor promoter in advanced prostate cancer (PCa). We speculated that microRNAs (miRNAs) that are inhibited by TGF-β1 might exert anti-tumor effects. To assess this, we identified several miRNAs downregulated by TGF-β1 in PCa cell lines and selected miR-3691-3p for detailed analysis as a candidate anti-oncogene miRNA. miR-3691-3p was expressed at significantly lower levels in human PCa tissue compared with paired benign prostatic hyperplasia tissue, and its expression level correlated inversely with aggressive clinical pathological features. Overexpression of miR-3691-3p in PCa cell lines inhibited proliferation, migration, and invasion, and promoted apoptosis. The miR-3691-3p target genes E2F transcription factor 3 (E2F3) and PR domain containing 1, with ZNF domain (PRDM1) were upregulated in miR-3691-3p-overexpressing PCa cells, and silencing of E2F3 or PRDM1 suppressed PCa cell proliferation, migration, and invasion. Treatment of mice bearing PCa xenografts with a miR-3691-3p agomir inhibited tumor growth and promoted tumor cell apoptosis. Consistent with the negative regulation of E2F3 and PRDM1 by miR-3691-3p, both proteins were overexpressed in clinical PCa specimens compared with noncancerous prostate tissue. Our results indicate that TGF-β1-regulated miR-3691-3p acts as an anti-oncogene in PCa by downregulating E2F3 and PRDM1. These results provide novel insights into the mechanisms by which TGF-β1 contributes to the progression of PCa.

ABSTRACT

In China, about 100 million people currently have chronic obstructive pulmonary disease (COPD). At the same time, COPD is a multisystem disease, not only affecting the function of musculoskeletal, cardiovascular, kidney and immune systems in patients, but also causing intestinal dysfunction as its extrapulmonary manifestations. From the perspective of traditional Chinese medicine (TCM), after COPD is formed, deficiency, phlegm stasis and toxicity were accumulated in the lungs, which leads to dysfunction of lung in dispersing and descending, and eventually causes ascending and descending disorder of Qi activities, disorder of fluid supply and distribution, and stagnation of blood stasis. The viscera disease would affect the bowels, and the large intestine is thus affected. Modern medical discovers that, the lungs and intestines have common origins and similar physiological structures, in pathological circumstances, their common mucosal immune system may lead to similar immune factors and inflammatory manifestations in the lungs and intestines. At the same time, the studies have confirmed that there is also a close relationship between intestinal flora and lung, that is "lung-gut axis". These theories partially illustrate the mechanism of COPD in inducing intestinal injury. The specific manifestations of COPD intestinal dysfunction, ① Flora disorder, with increased abundance of intestinal gram-negative bacilli, and inhibited reproduction of Bifidobacterium, Lactobacillus and short-chain fatty acid-producing bacteria. ② Intestinal barrier damage: characterized by the destruction of intestinal epithelium tight connectivity, increased intestinal permeability, and thinning of the mucus layer. ③ Intestinal motility disorder: mostly manifested as weight loss and malnutrition. At present, for the intestinal dysfunction in COPD patients, most of the relevant discussions and targeted treatment methods in TCM are scattered and unsystematic. Guided by the idea of treating different diseases with the same treatment, we summarized the etiology and pathogenesis of COPD intestinal dysfunction by learning from the experience of TCM in treating intestinal flora disorders and inflammatory bowel disease, and proposed preliminary formulation with Tiaoqi Qushi,Tongfu Tongluo as its basic treatment principles in this paper, hoping to provide new ideas for the treatment of COPD.

ABSTRACT

OBJECTIVE@#To analyze the clinical efficacy of hip arthroplasty with femoral calcar prosthesis and proximal femoral nail fixation(PFNA) in the treatment of elderly patients(≥80 years old) with unstable intertrochanteric fractures(Evans Ⅲ, Ⅳ).@*METHODS@#From June 2016 to March 2018, 60 elderly patients with unstable intertrochanteric fractures treated with prosthetic replacement and PFNA were retrospectively analyzed. According to the surgical methods, they were divided into PFNA group and prosthesis group. In PFNA group there were including 21 males and 15 females, with an average age of(84.3± 2.9) years old;in the prosthetic group, there were 10 males and 14 females with an average age of (82.9±2.4) years old. The operation time, hemoglobin difference between preoperative and postoperative 1 day, postoperative ambulation time, hospitalization time and complications were observed and compared between the two groups. Harris hip score was performed 3 and 12 months after operation.@*RESULTS@#All patients were followed up for 12 to 24 months (19.3±4.8) months. One patient in the prosthesis group died of lung cancer one year later and the follow-up was terminated. The operation time of prosthetic group was longer than that of PFNA group(@*CONCLUSION@#The elderly patients with intertrochanteric arthroplasty can reduce the burden of intertrochanteric arthroplasty and improve the quality of life.

ABSTRACT

Purpose To study the expression and the clinical significance of β3GnT8,MMP-2,PCNA in gastric cancer tissue. Methods The histological chips of paraffin specimens of gastric cancer were prepared,and immunohistochemical methods were used to detect the expression of β3GnT8,MMP-2 and PCNA in gastric cancer and adjacent tissue. Results The expression of β3GnT8 and MMP-2,PCNA in gastric cancer was higher than that in adjacent tissue (P = 0. 001) . The expression of β3GnT8 and MMP-2 was significantly positively correlated with the clinical stage (P = 0. 001) ,depth of invasion (P = 0. 011) and lymph node metastasis (P = 0. 003) of gastric cancer. The expression of β3GnT8 was significantly positively correlated with that of MMP-2 (r = 0. 703,P = 0. 001) and PCNA (r = 0. 231,P = 0. 024) . The overall survival time of the β3GnT8 positive expression group was significantly shorter than that of the negative expression group(χ2 = 3. 957,P = 0. 047) . Conclusion The expression of β3GnT8 is increased in gastric cancer. β3GnT8 can promote the invasion and metastasis of gastric cancer and is associated with poor prognosis in patients with gastric cancer.

ABSTRACT

Clinical characteristics of Yinhua Miyanling Pian user group were analyzed based on real world hospital information system (HIS)database. The information was collected from the HIS in sixteen hospitals of grade Ⅲ-A. Normalizing the data and descriptive analysis was performed. Among the 5 312 cases, female patients(63.67%) were more than male patients (36.33%). The median age was 49 years old. The age of 18-65 accounted for 74.52%.The patient was admitted to the hospital's department of obstetrics and gynecology(28.71%) and urology(28.43%). The median single dose is 2 g, accounted for 49.55%. The median daily dose is 6 g. 88.80% of patients were treated for less than 7 d. The median hospitalization were 12 d, most were 7-14 d accounted for 41.70%. Most hospitalization expenses payment by medicare, accounted for 80.22%. The median hospitalization expenses was 12 211.47 RMB. Most patients with benign tumor(27.36%) and malignant tumor(15.56%), next is the obstruction of urinary tract(15.49%) and urinary calculi(10.52%). The most common syndromes were damp heat syndrome(32.46%), liver and kidney deficiency syndrome(15.33%) and splenasthenic fluid-retention syndrome(15.01%). Clinical use is combined with antibiotics, as well as traditional Chinese medicine heat clearing agents, tonic drugs and so on. Finally, 44.22% were cured and 51.05% were better. Most of the drug users were adults, and mostly were female. Most with the tumor, urinary tract obstruction or stones. TCM syndrome is mainly characterized by dampness and heat, deficiency of liver and kidney, and dampness of spleen. In clinical practice, it was mainly combined with Western medicines, and Chinese medicines were also a-vailable. And most are combined with antibiotics.The medication basically conforms to the drug instruction. Based on the results of the real world HIS,Yinhua Miyanling Pian could provide theidea and reference for regulating the medication in adolescent patients.

ABSTRACT

<p><b>BACKGROUND</b>Associations between glutamine (Gln) enriched nutrition support and surgical patients with gastrointestinal (GI) tumor remain controversy. The purpose of this meta-analysis was to assess the effect of Gln enriched nutrition support on surgical patients with GI tumor in term of relevant biochemical indices, immune indices, and clinical outcomes.</p><p><b>METHODS</b>Six databases were systematically searched to find eligible randomized controlled trials (RCTs) from 1966 to May 2014. When estimated the analysis indexes, the relative risk (RR) was used as the effect size of the categorical variable, while the weighted mean difference (MD) was used as the effect size of a continuous variable. Meta-analysis was conducted with Rev Man 5.2.</p><p><b>RESULTS</b>Thirteen RCTs, involving 1034 patients, were included in the meta-analysis. The analysis showed that Gln enriched nutrition support was more effective in increasing serum albumin (MD: 0.10; 95% confidence interval [CI]: 0.02-0.18; P < 0.05), serum prealbumin (MD: 1.98; 95% CI: 1.40-2.55; P < 0.05) and serum transferring (MD: 0.35; 95% CI: 0.12-0.57; P < 0.05), concentration of IgG (MD: 1.26; 95% CI: 0.90-1.63; P < 0.05), IgM (MD: 0.18; 95% CI: 0.11-0.25; P < 0.05), IgA (MD: 0.22; 95% CI: 0.10-0.33; P < 0.05), CD3 + (MD: 3.71; 95% CI: 2.57-4.85; P < 0.05) and CD4/CD8 ratio (MD: 0.27; 95% CI: 0.12-0.42; P < 0.05). Meanwhile, it was more significant in decreasing the incidence of infectious complications (RR: 0.67; 95% CI: 0.50-0.90; P < 0.05) and shortening the length of hospital stay (MD: -1.72; 95% CI: -3.31--0.13; P < 0.05).</p><p><b>CONCLUSIONS</b>Glutamine enriched nutrition support was superior in improving immune function, reducing the incidence of infectious complications and shortening the length of hospital stay, playing an important role in the rehabilitation of surgical GI cancer patients.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To systematically assess the effect of early enteral nutrition support after gastrointestinal operation on prognosis.</p><p><b>METHODS</b>The Cochrane Library, PubMed, CBM, CNKI, Wanfang, and VIP databases were retrieved via computer system for randomized controlled trails(RCTs) with early enteral nutrition support to patients undergoing gastrointestinal operation. Quality of studies was evaluated by the Cochrane Jadad rating scale. Nutrition indexes, bowel function indices, postoperative complications, health-economics indices were collected. Meta-analysis was conducted with RevMan 5.2.</p><p><b>RESULTS</b>Eleven relevant RCTs studies with 1087 cases were enrolled, including 541 patients in the study group(early enteral nutrition) and 546 in the control group. Meta-analysis showed that patients in the study group had significantly higher levels of plasma albumin and prealbumin than those in the control group(WMD=2.87, 95%CI:1.03-4.71; WMD=0.04, 95%CI:0.02-0.05). The time of postoperative bowel ventilation in the study group was significantly shorter than that in the control group(WMD=4.10, 95%CI:-5.38--2.82). The postoperative complication rate in the study group was significantly lower as compared to the control group(RR=0.64, 95%CI:0.44-0.93).</p><p><b>CONCLUSION</b>Early enteral nutrition support after gastrointestinal operation is safe and effective, which can improve the nutritional status, promote bowel function return, and reduce postoperative complication rate.</p>

ABSTRACT

ObjectiveTo investigate the application of rapid immunohistochemical staining technique in intraoperative frozen section diagnosis of thyroid neoplasm.Methods MaxVision one-step rapid immunohistochemical staining technique was used to detect the expression of CK19,HBME-1,and Gal-3 in frozen section of papillary thyroid carcinoma(PTC)andthyroid benign lesions.MaxVision conventional immunohistochemistry of frozen remaining tissue was served as control.ResultsMaxVision one-step rapid immunohistochemical staining technique could be completed in 20 minutes.The positive localizations of three markers detected by rapid immunohistochemistry were similar to conventional immunohistochemistry, in general.The expression of CK19 was located in cytoplasm and cellular membrane.Gal-3 and HBME-1 were mainly detected in follicular luminal border and/or surface of papilla. The staining intensity in rapid immunohistochemistry was stronger than that in conventional immunohistochemistry. The positive rates of CK19,HBME-1,and Gal-3 by rapid immunohistochemistry in frozen sections were: 0 (0/28),10.7 % (3/28),0 (0/28),respectively,for benign lesions (nodular goiter,Hashimoto thyroiditis,thyroid adenoma); and 94.9 %(37/39),92.3 % (36/39),92.3 % (36/39),respectively,for PTC.The expression of three markers between thyroid benign lesions and PTC had a significant difference (x2 =59.326,55.861,44.605,all P < 0.001).In benign lesions,the rate of same case with two and more positive markers was 0,while in PTC it was 100 % and significantly different (x2 =67.000,P < 0.05).ConclusionMaxVision one-step rapid immunohistochemical staining technique could be applied in intraoperative frozen section diagnosis.Detecting CK19,HBME-1,and Gal-3 expression in intraoperative frozen section has an auxiliary value for diagnosis of PTC.

ABSTRACT

Objective: To explore the therapeutic effect of total parenteral nutrition(TPN) combined with enteral nutrition (EN) in treatment of severe acute pancreatitis (SAP). Methods: A total of 126 patients with SAP were equally randomized into the following three groups: TPN group, EN group, and TPN+ EN group. Peptisorb enteral nutrition was chosen and was given by jejunostomy or nasojejunal feeding tube. The changes of clinical parameters were observed in the 3 groups. Results: The APACHE- II scores, Ranson score, and the concentration of C reactive protein were not significantly different between the 3 groups before treatment. The total bilirubin in TPN group was significantly increased after treatment (P<0.05); the curative rate, mortality rate, plasma albumin, prealbumin, urea nitrogen, creatinine, cholesterol, triacylglyceride, and blood glucose underwent no significant difference. Compared with TPN and EN groups, the TPN + EN group had a shorter hospital study, lower rate of infectious complications (P<0.05). Compared with TPN group, EN group had a significantly lower rate of infectious complications (P<0.05). Conclusion: The combined application of TPN and EN can effectively reduce complications and decrease the average hospital stay in SAP patients; the method is more suitable for SAP patients.

ABSTRACT

Objective: To investigate the influence of alanyl-glutamine-enriched enteral nutrition on the intestinal permeability in patients with abdominal surgical stress response. Methods: A total of 216 patients undergoing abdominal surgery were randomly divided into three groups, namely, the conventional alanyl-glutamine-enriched enteral nutrition (GLN group), enteral nutrition (EN group) and parenteral nutrition(PN group). The three groups were given isonitrogenic (0.2g · kg-1 · d-1) and isocarloric (125.4 kJ · kg-1 · d-1) support during 3-11 days after operation. The urinary lactulose/mannitol (L/M) ratios were observed 6 h after the patients were given 10 g lactulose and 5 g mannitol on one day before and the 7th, 12th day after operation. High pressure liquid chromatography was used to determine the urinary levels of lactulose and mannitol. Results: The mannitol levels had no significant change before and 7, 12 days after operation in the 3 groups, and there were also no significant difference between the 3 groups before and 7, 12 days after operation. The L/M values before and 7, 12 days after operation were 0.025±0.009, 0.038±0.017, and 0.028±0.012 in GLN group, 0.024±0.011, 0.061±0.019, and 0.027±0.014 in EN group, and 0.026 ± 0.012, 0.083 ± 0.021, and 0.047 ± 0.013 in PN group, respectively. Statistical analysis showed that the L/M ratios in the 3 groups were all significantly increased 7 days after operation compared with those before operation (P<0.05), and that of PN group was significantly higher than those of EN group and GLN group (P<0.05), and that of EN group was significantly higher than that of GLN group (P<0.05). Twelve days after operation, the L/M ratios in PN group was still significantly higher than that before operation and those in the EN and GLN groups (P<0.05), and that in EN group was similar to that before operation and that in GLN group. Conclusion: The intestinal permeability is greatly increased during the early period after operation. Enteral nutrition is favorable for maintaining the function of intestinal mucosal barrier. Alanyl-glutamine-enriched enteral nutrition can improve the function of intestinal mucosal barrier.

ABSTRACT

Objective: To study the effect of Cordyceps sinensis and Panax notoginseng compound extracts (CSPNE) on Bleomycin-induced pulmonary fibrosis (PF) in rats and its mechanisms. Methods: Wistar rats were randomly divided into five groups (18 rats in each group). Rats in model, high- and low-dosage (150 and 75 mg/kg) CSPNE, and Prednisone acetate (3.33 mg/kg) groups were injected with a single dose of Bleomycin hydrochloride through trachea and in control group with the same volume of normal saline. One day after the injection, CSPNE solution of different dosages was respectively ig given to rats daily, while the same volume of normal saline was given to those in the control and model groups. On day 7, 14, and 28 after medication, six rats in per treatment group were randomly killed. Lung samples in every group of rats were measured for lung index, hydroxyproline (Hyp) and malonaldehyde (MDA) contents, and superoxide dismutase (SOD) activities, and serum was analyzed for SOD activities and MDA contents. Lungs were incised to make pathological sections which were stained with HE, and the expression of TGF-β1, Collagen I, and Collagen III in lung tissue was analyzed by means of immunohistochemistry technique. Arterial blood gas detection was observed in every group. Results: CSPNE could significantly decrease the lung index and Hyp contents in the lung tissue of rats with PF. It could enhance SOD activities and reduce MDA contents both in serum and lung tissues. It also decreased the expression of TGF-β1, Collagen I, and Collagen III in lung tissues. In the arterial blood gas detection CSPNE was found to make [p(CO2)] lowered, while [p(O2)] increased in the artery of rats with PF. Conclusion: CSPNE has obvious effect of inhibiting PE of rats, and the mechanism may be associated with its antilipid peroxidation, inhibiting the expression of TGF-β1, reducing the alveolar interstitial collagen deposition, and improving respiratory function as well.

ABSTRACT

<p><b>OBJECTIVE</b>To determine the effects of diazoxide on oxygen free radicals and cell apoptosis in brain tissue after deep hypothermia cerebral ischemia reperfusion injury in young rats.</p><p><b>METHODS</b>Fifty-four 3-week-old Sprague-Dawley rats were randomly and equitably divided into sham-operated group, model group and diazoxide group respectively (n = 18). The model of hypothermia cerebral ischemia reperfusion injury was made. After 24 hours of operation, the brains of rats were removed and preserved. The content of superoxide dismutase (SOD) and malonaldehyde (MDA) in brain tissue were detected. Cytosolic C release of cytochrome was confirmed by Western Blot. The protein expression of Caspase-3 was determined by immunohistochemistry.</p><p><b>RESULTS</b>In the model group, the content of SOD was (198 +/- 41) U/mg, lower than the sham-operated group's (321 +/- 36) U/mg (P < 0.01). The content of MDA was (212 +/- 21) nmol/mg, was higher than the sham-operated group's (100 +/- 23) nmol/mg (P < 0.01), and the expressions of cytochrome C (0.72 +/- 0.09) and Caspase-3 (83 +/- 10) were all significantly higher than those in the sham-operated group (0.17 +/- 0.02 and 115 +/- 9) (P < 0.01). Compared with the model group, the content of SOD in the diazoxide group [(264 +/- 34) U/mg] was markedly increased (P < 0.05). In addition, diazoxide provided significant reductions in the content of MDA [(174 +/- 19) nmol/mg] and the expressions of cytochrome C (0.41 +/- 0.05) and Caspase-3 (99 +/- 11) (P < 0.05).</p><p><b>CONCLUSIONS</b>The neuroprotective effects of diazoxide against brain injury induced by deep hypothermia cerebral ischemia reperfusion through inhibiting oxygen free radicals and cell apoptosis. Diazoxide may become a new neuroprotective drug after infant complicated congenital cardiac operation.</p>

<p><b>OBJECTIVE</b>To investigate the effect of the vector carrying short hairpin RNA targeting epidermal growth factor receptor (shRNA-EGFR) on the radiosensitivity of human nasopharyngeal carcinoma xenografts in nude mice.</p><p><b>METHODS</b>shRNA-EGFR was transfected into human nasopharyngeal carcinoma cell line CNE1 via Lipofectamine 2000. The transfected cells were collected for quantitative RT-PCR detection of the expression level of EGFR mRNA. Western blotting was used to examine the expression of EGFR protein. CNE1 cells were inoculated into nude mice and the tumor volume was measured every 2 days. shRNA-EGFR was intratumorally injected in the mice, and 16 days after radiotherapy, the mice were sacrificed and tumors examined for radiosensitivity.</p><p><b>RESULTS</b>shRNA-EGFR was effectively delivered via Lipofectamine 2000 into CNE cells to result in a significant downregulation of EGFR mRNA and protein expressions (P<0.05). A significant difference was noted in the tumor volume and weight in the tumor-bearing nude mice between shRNA-EGFR plus radiotherapy group and the control, exclusive radiotherapy and shRNA-EGFR groups (P<0.05).</p><p><b>CONCLUSION</b>shRNA-EGFR combined with radiotherapy can effectively inhibit the growth of nasopharyngeal carcinoma in nude mice. shRNA-EGFR can enhance sensitivity of nasopharyngeal carcinoma to radiotherapy.</p>

<p><b>OBJECTIVE</b>To investigate the effects of diabetes mellitus on amplitude of distortion product otoacoustic emissions ( DPOAE) and whether streptozotocin has ototoxicity or not.</p><p><b>METHODS</b>Twenty-four SD rats were divided into diabetes mellitus group, insulin therapy group and control group. Diabetes was induced by streptozotocin. The blood glucose of insulin group was controlled by insulin. The amplitude of DPOAE was tested before streptozotocin injection and 3 months, 5 months after injection, respectively.</p><p><b>RESULTS</b>The amplitude of DPOAE in diabetes rats was decreased in 3 months and there was statistical significance (P <0. 05) in 1038, 4126 and 5200 Hz, while obviously reduced in 5 months in all frequencies and there was also statistical significance (P <0. 01). The amplitude of DPOAE in all frequencies lacked of any significance between insulin therapy group and control group.</p><p><b>CONCLUSIONS</b>Diabetes mellitus could induce the amplitude of DPOAE decreasing. Streptozotocin had not evident toxicity for inner ear.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the expression of Bcl-2 and Bax in rat liver fibrosis model induced by CCl4 and the role of IFN-gamma.</p><p><b>METHODS</b>Liver fibrosis was induced in rats by subcutaneous injection of CCl4. The rats were divided into fibrosis model group, Bie-Jia-Ruan-Gan-Tablet treatment group and IFN-gamma treatment group (0.2 MU.kg.d, i. m. for 12 weeks), and another 10 rats without any treatment were used as normal control. Bcl-2 and Bax proteins expression were detected by immunohistochemistry.</p><p><b>RESULTS</b>Bcl-2 was expressed weakly in the homogenate of hepatocytes and hepatic sinusoid in normal control rats, and it was expressed stronger in fibrous septae, portal area, hepatic sinusoid, the homogenate and membrane of hepatocytes and central vein in fibrosis model rats (3.87%+/-2.37% vs 9.46%+/-4.29%, t=2.83, P<0.05). Bax was expressed slightly in central vein and the hepatic sinusoid around, and it was expressed stronger in the homogenate of hepatocytes, hepatic sinusoid, fibrous septae, membrane of hepatocytes and epithelial cells of bile duct (3.50%+/-1.88% vs 9.80%+/-3.75%, t=3.72, P<0.01). Compared with that in fibrosis model rats, the expression of Bax was significantly lower in rats treated with IFN-gamma (9.80%+/-3.75% vs 5.85%+/-2.35%, t=2.98, P<0.01), but the expression of Bcl-2 was not significantly different (t=1.49, P>0.05), however it was significantly lower in fibrous septae in IFN-gamma-treated group than in model group (6.58%+/-4.13% vs 9.46%+/-4.29%, t=2.80, P<0.05).</p><p><b>CONCLUSION</b>The expression of Bcl-2 and Bax increases in liver fibrosis model rats, and IFN-gamma can promote myofibroblasts apoptosis</p>
